Dear Coach:

Thank you for your interest in our Licensure Program.

There are two ways for you to become licensed to use the "Contacts Count"© approach and materials with your clients. Either way, first print out this Application Form and the Licensure Agreement, fill them in and sign them as noted. Then, either:

1. Mail the two documents along with your check for $650 ($500 for materials + $150 for the first year)   OR
2. Scan and email the two documents to LWaymon@ContactsCount.com AND pay your fee using this PayPal link:   Upon receipt, we will notify you of your acceptance and send the materials to you via email and regular mail. We’ll also schedule your orientation session; it’s designed to help you maximize your success with the materials.
